Assessment Procedure

Service User’s will have an identified learning disability and/or a sensory impairment or Autism they must be over seventeen.  

Rhombus Care Group will consider offering placements to Service User’s with a criminal record but will consider each on individual basis. Everybody’s safety will be a priority at all times.

Rhombus Care Group will complete a care plan for each person that lives in a Rhombus care Group house and all plans will incorporate each persons’ needs, religious and cultural identity. Such plans will reflect variations in diet, dress and any other requirements. The framework used by the agency is drawn from Supporting Peoples “Outcomes Framework” and covers the following outcomes.

Upon receiving a referral, we will request sufficient information to ascertain whether your referral falls broadly in line with any of our stated criteria.

If outside the criteria we will be unable to offer you a placement, and will explain the reasons for this to you in full, we will suggest an alternate provider more suited to meeting your needs. Should you disagree with this outcome we also offer the right to appeal the decision.
 
If there is a possibility that Rhombus Care Group could meet the stated needs we will in each case seek to follow these steps with you and or your representative:

 We will contact the Care Manager and arrange an informal meeting with you, your Family and any other significant party you would like to invite; and discuss what you want and need for the future. The most important thing about offering you a place is that it is what you want.

We will arrange an informal social outing with you and spend some time doing things you like. This will give you a chance to see if you like our staff also.

We will arrange for you to come and see our houses and meet some of the people that live there also to give you an experience of what living with Rhombus Care Group would be like.

We may arrange for you to be introduced to the service slowly and you may spend some overnight stays at your new house first.
